You can contact us by:
– phone call
– in person
– online
Receptionist / Care navigation
If phone call or in person request, the Receptionist/Care Navigator will capture this on a form. If an online form is used this goes to next steps of the triage process.
Team reviews request and will either…
– Signpost patient to the relevant local services as per modern general agreements.
– Signpost the request and patient to the relevant clinician in the practice for a call/appointment.
– Adds to a list for the Doctor (GP) to review…
– Contacts other services to further support patient care (e.g. District Nurses, Midwives, Health Visitors, etc…)
Assessment / Response
Doctor (GP) reviews the list and will take relevant action of:
– Doctor requests the Care Navigation Team to go back to patient with relevant message/advice.
– Doctor replies to the patient by text.
– Doctor calls the patient to either resolve the request or book them in for a face-to-face consultation.
All pathways are overseen and reviewed by your Doctors.
